• Plan of care is prepared based on the treatment intentions of your physician or podiatrist.
• Skilled nurse works with your doctor to provide medical care.
• Therapist helps with rehabilitation in order to restore safe independent functioning.
• Social Worker assists with various community resources including meals on wheels, homemaker services and transportation.
• Home Health Aide helps provide personal care.
• If the doctor orders dietitian services a referral is made suitable to your dietary needs.
• Specimen for blood work ordered by the physician is drawn at home and process in one of our associate laboratories.
•  We facilitate home delivery of medical equipment like hospital bed, wheelchair, walker, cane, grab bar, shower chair, incontinent supplies, oxygen, respiratory equipment, blood sugar testing machine and regular delivery of test strips, lancets, alcohol pads, and insulin syringes.
